PRESCRIPTIONS:
Remember, our Rx plan allows you to save money when you buy in 90 day supplies.
Some of our members who have Workman’s Comp related high blood pressure try to obtain their high blood pressure medications by using their UHC/MEDCO card. This is strictly prohibited and costs all of our member’s money. UHC/MEDCO will automatically reject these types of medicines. If you have a pre-existing cardiovascular condition and believe that the Health Trust should be paying for these meds for you, you must contact Chairman Kluger and provide the pertinent information so that arrangements can be made to cover those medications.
Workers Comp:
Remember, under workers comp statutes, (State Law) you are required to see your workers comp physician yearly. If you don't, you will lose your workers comp coverage. The Health Trust WILL NOT cover you for your workers comp problem.
Do not to use your UHC RX card for workers comp claims.
The Drop Plan and Retirement:
Remember, the Plan requires that in order to use the Health Trust after retirement, you must have been a member of the Health Trust for period of four years, prior to retirement.
Remember, the Health Trust pays for a full cardiac physical 6 months prior to the DROP Plan and again 6 months prior to final separation. We strongly encourage all our members to take advantage of this great benefit.
Change of Status:
You must notify the Health Trust by calling Administrative Services (305-595-4040) for any change in status of your coverage within 30 days:
Open enrollment:
Open Enrollment commence about November 01,each year. You will have 1 month to make changes in your coverage (add people to be covered or drop members) that were not the result of qualifying events. Changes will be effective January 01, 2008.
Dependent Children:
Every year members must submit documentation to the Trust to show that their children over 18 years of age are still “dependent.” This affects those with children who are still attending school or otherwise are fully dependent and living at home. Failure to comply with these notices will result in the dependent being terminated from the Health Trust. Coverage for children is automatically terminated at age 25 whether or not they remain dependant.
